This educational writing is designed to help you learn about all the important elements that go into the technical aspects of the operation. By understanding how the operation is actually performed, you will have a better understanding of some of the essential aspects of the eyelid lift. This educational writing was done at the request of a patient of mine from Philadelphia, PA. A: Overview A blepharoplasty is an operation to help improve the appearance of the eyelid area. As we age, we develop hooding, bags, and circles around the eyes. The operation is done to reverse these age associated changes. By doing the procedure, we become more refreshed and youthful in the eye area. You can watch me perform an eye tuck operation by clicking on this link. It's a very cool video that most find interesting, and it's very delicate and precise. B: Upper Eyelid Lift or Eye Tuck Upper eyelid surgery is typically done for excess skin and fullness in the upper eyelids. The excess skin is called hooding, and the medical term is dermatochalsis. The eye lift is done with an incision in the eyelid crease. This is the line above your upper eye lashes, and it is located about 6-12 mm above the lashes. It also is called the supratarsal crease. The excess skin that occurs with aging often covers and hides the eyelid crease. So when woman wear make-up, the crease can be covered or blurred, and eye shadow doesn't work and smudges. The incision for the eye tuck is typically placed right in this crease. By placing it right in this crease or location, we know we will usually have extremely good fading of the incision as part of the result. B1: Technique The operative technique I use addresses all 3 important layers including the skin, muscle, and fat. I believe this is the most advanced way to perform this operation. Some doctors just remove the excess skin during their eye tuck. This leads to results that are not as good as when all 3 layers are treated. The skin excess is meticulously measured, and the excess is removed with delicate dissection scissors. The next layer addressed is the muscle layer. This muscle is called the obicularis oculi. It is the muscle that can cause crows feet and also closes the eye. It is inervated by the 7th cranial nerve called the zygomatic branch of the facial nerve. Excess and aging muscle is then trimmed or removed to enhance the result. B2: The Fat Finally, the fat layer is treated. There are three compartments of fat in the upper eyelid area. These fat compartments reside behind the orbital septum. Excess fat can give us a baggy and tired look. The excess fat is removed to complete this part of the blepharoplasty. Care is taken to remove the correct amount of fat and not too much. Too much fat removal can lead to a hollowed out look. This complication is rare, and I am meticulous in removing just the correct or right amount of fat. B3: Surgical Closure The entire area is then examined to make sure there is no bleeding. This is called checking for hemostasis. An electro cautery devise is used that is called a Bovie. After all bleeding is stopped, closure is done with a tiny stitch that is blue in color. It is called a Prolene stitch, and it  is finer than a human hair. I use a delicate closure technique that is called a subcuticular stitch, so everything is inside, except a small part that allows it to be removed. It's also called a "pull-out" stitch because it is so delicate and has almost no pain with removal. C: Lower Eyelid Lift or Eye Tuck The lower eye lift procedure is very similar to the upper one. The operation is done with a tiny incision just beneath the eye lashes of the lower lid. I actually place the incision right beneath the lashes so it is extremely difficult to see when healed and the recovery is complete. Just like the upper procedure, the goal is to address all 3 layers of aging to get the best result. This includes excess skin, muscle, and fat. C1: Fat The fat of the lower eyelid gives the bags and circles that is the main contribution to looking old and tired in this area. The fat bags come in 3 discrete compartments in the lower eyelid. These are called the medial, central, and lateral fat compartments. Great care is taken in removing these fat pockets. The lateral or outside pocket is missed by the surgeon in up to 10 % of cases. I work meticulously to treat this pocket and to remove just the correct amount. Excess skin and muscle is removed as described above. Great care is taken to remove the best amount of tissue - not too little and not too much. This is part of the artistic aspect of the blepharoplasty or eye tuck operation. Closure of the incision is again with the delicate Prolene "pull-out" stitch. C2:  The Transconjuctival Lower Eye Tuck The transconjunctival lower eye tuck is a special method or technique that I perform. It is an advanced surgical technique that many doctors do not use. The operation performs the eye tuck without any outside incision and just an incision inside the eyelid. The incision is placed in the conjunctiva of the eye. This is the inside layer. Through this location excess tissue is treated as described above. The fat pockets are exposed by going through the capsulopalpebral fascia of the lid. Closure is then done with a very delicate and absorbable stitch that dissolves in just days. This technique has its advantages over the standard technique. This approach leads to less scar tissue in the eyelid and a lower risk of eyelid malposition than more conventional procedures. A disadvantage of this technique is that no excess skin is removed with the transconjunctival approach. Laser treatment or peels can be done to help with excess skin issues if needed. This is sometimes called a laser blepharoplasty. I work closely with my patients to determine, based upon anatomy and other factors, whether this operation is correct or best for them. D: Summary The blepharoplasty or eye lift is a fantastic operation that helps with aging around the eyes. By learning about the different methods and how the operation is done, we get a better understanding of the cosmetic operation and its benefits. The operation is performed on an outpatient basis. Most people are surprised at how little pain is involved. Many of my patients will take no narcotic pills and maybe just Tylenol or Advil after surgery. Resolution of bruising and swelling does take some time, and this must be allotted for in the recovery period. Results from the procedure are very successful. Most people are extremely happy with the operation and feel much younger and more youthful. Medically, it is called a blepharoplasty. However, there are many other names for this cosmetic procedure such as eye lift or eye tuck and eyelid lift or tuck. They are done to improve and correct aging that occurs around the eyes. Common changes include hooding, bags, circles, and shadows that can cause a tired and old appearance to the face. This educational writing will focus on the essentials of this plastic surgery operation. This writing was done in response to a question from a patient who lives in Media, PA. Overview An eyelid lift is one of the most commonly performed operations by me and by plastic surgeons across the country. The American Society of Aesthetic Surgeons recently listed it as one of the top 5 procedures done each year. Reasons for the popularity of this operation include its high safety and success in delivering results that are desirable. It is one of my favorite operations, and I perform it extremely frequently. For those interested, you can watch me do this procedure by clicking here. In the video, I'm performing a lower blepharoplasty. It is a delicate operation with meticulous technique. Why and How are Eye Lifts Done As we age, visible changes are often seen around the eyes. This will commonly be some of the first signs of aging, before other signs of facial aging such as jowls, jaw line issues, or neck skin excess. We look at each other in the eyes, and so it's common to be very concerned about aging in this area. The main changes that occur in the eyelids are hooding of the upper lids and bags and circles in the lower lids. Surgery for the upper lids is called an upper blepharoplasty. Surgery on the lower eyelids is called a lower blepharoplasty. Patients can do just the upper eye lift or just the lower eye lift, depending on their desires. Many people will do both at the same time. The picture above shows the common fat pads that contribute to the aging process. The picture also shows the location of the incisions for the blepharoplasty operation. Anesthesia The operation takes about one hour. It can be done with different methods of anesthesia. Many people prefer sedation or general anesthesia. This is extremely safe and gives the added comfort of having no pain or memory of the procedure. Another choice is local anesthesia. This means lidocaine injections for the incisions. This has the advantage of not being put to sleep. It does require the person to be very still and to tolerate some pain. It is not for everyone, however. Most people prefer to go to sleep or get twilight sedation. Surgical Technique The operations are done with very small incisions around the eyelids. They are very delicate and fine. These incisions typically heal extremely well and are often hard to even see. The way these incisions heal so well is amazing to me. I would say this area heals the best of any area on the human body. It is why we can do the eye tucks, because the scars fade away to be typically imperceptible. The incisions are used to perform the eyelid lift. Hooding, bags, and circles are corrected through the incision. It is then closed with some of the most delicate of plastic surgery stitching. Recovery Recovery after a blepharoplasty is very easy. I can speak from person experience, since I have had both upper and lower eye lift operations. Pain is negligible. I didn't take a single pain pill, not even a Tylenol. Most patients are surprised at how little pain there is after this procedure. Some people will experience eye irritation and scratchiness. Bruising and swelling are normal parts of this recovery. This will take time to resolve. The majority of the bruising goes away over 7 - 10 days, but the final bruising can take a bit longer. Normal activities can be quickly resumed. Results Results after eye tucks are usually excellent and with high satisfaction. Most people are very pleased with their results and wish they had done it years earlier. The surgery frequently gives the person a more youthful and refreshed appearance around the eyes. The result is a less tired looking eye area and a more attractive and youthful look. It is also important to remember that cosmetic surgery improves us, but it doesn't turn us into movie stars. Realistic expectations about surgery is paramount to being happy after all cosmetic procedures. They don't turn us into a 20 year old or make us perfect. The procedures help us look younger and better, but not perfect. This is emphasized by me because having realistic expectations is a corner stone to understanding all procedures in cosmetic and plastic surgery. It involves the injection of a solution (Asclera) or sclerosing agent into the damaged veins which causes the walls of the vein to seal shut. Over time your body absorbs the damaged vein making it less visible. Based on the size of the area treated or number of areas a patient may require up to four treatments. Treatment is relatively painless and takes approximately 20-30min. Compression hose is worn after and strenuous activity is restricted for 3 days after. This educational writing was done in response to a question from an abdominoplasty / tummy tuck patient in Manhattan, New York.   Some people might say, "What do you mean by the term concepts?" The concepts are the guiding principles that relate to the surgery. It is the overview or the "big picture." Most concepts are not difficult to understand. If the doctor takes the time to explain them (as I do), they usually are clear and easily understood. An example might help demonstrate this point. This concept will focus on what results people can expect from plastic surgery for aging of the face. Patients wanting facial rejuvenation must learn to have realistic expectations. This is the idea that face lifts, eyelid tucks, and facial surgery can only make you look "so" young. A face lift can't turn you into a "movie star." When I tell patients this fact, most smile and respond, "of coarse not." But to really understand this concept is critical. It can be summarized by surgeons as expect "realistic expectations" from your procedure. It means that if you are 65 years old, surgery can't make you look 30 years old. It means surgery will "improve you" and not make you "perfect."   Some people will ask, "Why, can't surgery make me perfect." This is a reasonable question. The answer is that these procedures have limitations. They aren't magic. Only so much can be done. The world's best surgeon can only turn back the clock so much and so many years. Jowls, for example, can be improved. And this improvement can be substantial. But the expectation from the surgery and your results should be one of major lessening and decreasing of the jowls. Getting completely rid of them is unrealistic and is thinking similar to "Make me a Movie star or make me perfect." This concept is important to learn if you want a face lift or neck lift. It is this type of key learning point that you want to take away from a consultation. It is these types of facts that are important during your learning process about the surgery.   Let's look at another important learning concept. It involves instructions for recovery. You will be given detailed and thorough instructions after all operations. My patients get my personal cell phone number. Try not to get too stuck on one particular instruction. Try and become educated about the recovery process, and learn the guiding points. For example, imagine you want a tummy tuck- called an abdominoplasty.  If the doctor instructs you during recovery to, "Take it easy," then problems can occur if you don't do this. Problems can turn into complications. Surgery is serious business. Taking it easy is not a complicated topic. But if you don't do this, problems can occur. A patient might decide to workout too early and cause fluid buildup after a tummy tuck. Or maybe they exercise with too much weight and then muscles can be injured after the abdominoplasty. The concept or the idea is easy. Take it easy physically after surgery. Specifics and details are provided to help you. So, you will be told exactly when you can resume everything physically. But if you understand the principles, then often you can follow the rules better. In this example, the concept or rule is about resuming physical activity after surgery. Understanding this will help you in your recovery from the surgery. For patients interested in recovering from abdominal surgery, they may wish to link to an educational writing on this topic now.   A final example will discuss the idea that patients have options. Learn about your options and become educated about them. This is an important step for you to accomplish. During all my consultations, I focus on providing options. With breast augmentation or breast enlargement, people have an option about breast implant size. The guiding idea or concept is that the result or the "look" you get varies or changes with implant size. Large breast implants will make you look different than smaller implants. People will often say, "I don't want to look fake." So we discuss the different appearances you will have with different implants. Smaller and more natural implants give natural results. Big ones will give a faker and rounder result and look. There are many different sizes too. So, a woman can pick in-between sizes to get the best size for them. The concept is the result or the look for breast augment is dependent on the implant size. More natural looks come from natural implants. Fake results come from big or fake implants.   When learning about cosmetic surgery, remember to become as educated as possible about the procedure or operation that interests you.
