How a scar or incision heals depends on factors such as the surgeon, the scar location, the operation, time, and individual human differences. These factors are discussed in this educational writing and it’s importance to cosmetic surgery results and plastic surgery scars.

A patient of mine from West Chester, Pennsylvania came to me and asked, “Get rid of my scars.” You unfortunately can’t. Scars are permanent. If a person has a laceration or an operation, the incision turns into the scar, and this scar is permanent. It will fade, however! It will become less noticeable, but it will always be there. Understanding this process is very important. Plastic surgery incisions eventually turn into scars.

The factors involved in scar fading, incision healing, and surgical results will now be discussed.

1- The Surgeon:

Everyone wants the best doctor or surgeon to sew up their cuts or lacerations. This is true of their cosmetic operations too. Usually this is the plastic surgeon. Plastic surgeons have extensive training focused on the best techniques to maximize the fading and healing of scars. The techniques used include gentle handling of tissue, meticulous hemostasis, small- delicate stitches, and precise approximation or suturing of the tissues. It is normal to want the best doctor to fix your lacerations or to perform your cosmetic operation. This will play a role in your healing. The plastic surgeon will use his extensive training to maximize the healing and the fading of the incision to give the best result.

2- The Location:

In real estate transactions, there is a phrase used often – “Location, Location, Location!!”
This applies to scars and incisions also. If we get accidentally cut or suffer a laceration, we have no choice of the location or where the cut or scar occurs. However, with operations we have a choice. The location is critically important. Certain areas on the body heal better than others. For example, the face tends to heal extremely well. I call it a “privileged” location. Face lifts, eyelifts, and other cosmetic operations on the face use incisions that almost uniformly heal exceptionally well. The incisions in these areas are placed in “privileged” areas and usually are difficult to see after healing. It is a characteristic of these operations that the scars heal extremely well. If they didn’t, these operations would not be so popular.

However, other locations don’t heal as well as the face. The back, for example, doesn’t heal as well as the face. The scars or incisions in this area tend to be thicker and wider than often desired. The knee or shoulder can heal with wider or suboptimal scars. Some areas of the body do not heal as well as others. Location is critical.

The breasts, like the face, tend to heal very well. Cosmetic surgery operations on the breast often have very nice scar fading and results because of this tendency. This is true also for the incision for an abdominoplasty or tummy tuck. These operations tend to have surgical results with nice incisions and scars that fade well with time.

3- The Operation:

Different operations will have different results and scar healing patterns. As discussed above, certain operations use incisions that are not in the best locations. Thigh lifts, body lifts, buttock lifts have incisions that might not heal as well as other operations. A person might have to except a scar that is not as good as desired or thicker or wider than desired. These locations might have a higher incidence or chance of poor healing. Other operative factors can play a role. For example, operations that have complications, infection, or delayed healing will effect the scar or the surgical result. Delayed healing beyond three weeks is often associated with hypertrophic or keloid scars. Therapy is available if this happens, so this can help the fading if it occurs.

4- Time:

Scars or incisions fade with time. It can take much longer than many people realize to see complete fading of the scar. A good rule of thumb is that scars fade over a year. However, it can take longer for some people. Scars sometimes can take 18 months or even longer. It is important to be patient. It most cases the scar will fade with time. The great majority of people will ultimately be happy about the appearance of the scar. The final surgical result awaits complete fading of the incisions.

5- Individual Differences:

Scar fading is not the same for everyone. Personal differences in skin type, quality, and healing abilities can make a big difference in the final appearance. Patients might have genetic differences that lead to better or worse healing. Some patients will be more prone to keloid scarring. If you know this is the case for you, make sure you tell me as things can be done to enhance healing.

Summary:

Cosmetic surgery requires incisions. These incisions turn into the scars. The final result of the cosmetic operation occurs after the scars have faded to the greatest degree. Scars do not go away completely. They usually heal to the point where most people are very happy with the incisions. Factors that are involved in scar healing include but are not limited to: the surgeon, the location, the operation, time after surgery, and individual differences.

After an abdominoplasty, people often go down a size or two in clothing size – but not always! This change of clothing size is discussed in this educational writing. A tummy tuck operation can lead to a significant reduction in post op clothing size, in addition to other benefits of the surgery. Compression garments are recommended after the surgery and are reviewed in another writing.

People decide to have a tummy tuck to improve and enhance their abdominal or stomach area. The surgery helps with excess and loose skin, stretch marks, and extra fat. The results of the surgery is highly successful and can be dramatic at times. Please see the tummy tuck before and after photos now for those interested. Many patients will ask about the size change they will have after the procedure. Phrased another way, people ask about abdominoplasty change in clothing size. The answer is not always predictable. Some will have this change and others will not.

Clothing sizes are not standardized. A size 8 will vary from manufacturer to manufacturer. Some will run small and some big. European and American clothing can also be a variable. The same can be said for bra sizes. Manufacturing differences exist. A woman’s clothing size will also vary from her hips to her thighs to her stomach. Some women might be an 8 in the legs, but she might fit better in a 6 in the stomach areas.

This manufacturing difference in clothing, and variation in each women, makes it impossible to exactly predict the change after the surgery. Manufacturing of clothing was indeed standardized after WWII. However, this has long been abandoned. Companies have found it profitable to alter the “fit” or size to sell more clothing. For example, some women will buy a size 6 that decades ago would have fit a size 10. The women purchasing this clothing like the smaller size. It makes you feel better and hence more are sold.

Having said this, most women after a tummy tuck will go down a size or two. Some will go down even more! However, some patients will not have a change of size, and rarely, some will actually wear a larger size.

Most women will go down a size or two after an abdominoplasty and be very happy with their new appearance. The woman that doesn’t change sizes can unfortunately be disappointed by this. It is important to understand that even if you don’t go down a size, you still can enjoy the flatter and more sculpted stomach that the surgery achieves.

Women that wear a larger size can attribute this to swelling, if it is close to the time of surgery. If a significant time has past since the operation, than the answer lies in wearing the clothing differently. Some women will wear their pants slightly higher or lower after a tummy tuck. A large stomach roll might lead to someone pulling their pants above this “gut.” After surgery, since the stomach roll is gone, they might wear the pants in a different location, accounting for this size change. Once again, the stomach will “look” better and flatter despite the lack of dropping down in the clothing size.

To summarize, following cosmetic surgery tummy tuck, most women will go down a size or two in their clothing size. Some woman won’t have a change of size, and rarely some will actually go up in size.

Funny plastic and cosmetic surgery quotes, phrases, and expressions are reviewed in this educational writing. The writing is more for “fun” and “enjoyment” than any other reason. Doctors have their own language. It’s not just anatomy terms that might be unfamiliar, but doctors use expressions to convey concepts. These terms are sometimes funny or even peculiar. They are reviewed here for your amusement. Doctors can use words or terms that are difficult to understand. These expressions are meant to put concepts into easy to understand words. I hope you laugh and enjoy them!

1: ”Spitting Stitches”

Surgical incisions are sutured by the plastic surgeon or cosmetic surgeon with advanced skill and technique. This includes gentle tissue handling, meticulous control of bleeding, and fine delicate sutures. Many procedures have buried or “internal” stitches that absorb or dissolve with time. This allows for an incision with no visible stitches and can minimize the scarring. Expectations are nice healing and fading of the scar. This is called scar maturation or scar fading with time.

During this healing process, one might find that the incision “spits” or things come out of the incision. This is very common. Most sutures used are absorbable and placed in the skin. As the skin heals and turns over, the suture works it’s way to the surface. This can come out and is called a “spitting stitch.” It doesn’t hurt at all! But, it can be scary to some people. The stitch won’t affect the cosmetic result, so there is no reason to worry.

Other aspects of a healing incision can lead to the incision spitting. During the repair of the skin, sweat glands, oil glands, and hairs can get blocked. This can lead to minor infections. These usually are no more concerning than a pimple. So don’t worry. You usually see redness, swelling, and minor pain in a small part of the incision. Some pus drainage is not uncommon. Treatment is usual just Neosporin and band aides. This will not affect the cosmetic outcome.

2: ”Are You Eating, Drinking, Peeing, and Pooping?”

After surgery, patients are given specific and thorough instructions. A frequent question asked is, “What should I look for that might go wrong after surgery?” The standard answer is to look for high fevers (above 101.5 degrees), excessive bleeding, or excessive pain. My patients are given my personal cell phone for questions or concerns. However, if we check basic body functions and find they are well, we can then have a better assurance that all is well. Basic body functions are things like eating, drinking, urinating (peeing), and having bowel movements (pooping). If we are doing all these things, than we can often know that we are healing well after surgery.

This is not a guarantee that everything is perfect. It just means things appear to be going well, if they are all being done. It’s a good sign of progressing recovery.

3: ”Be the Turtle!” and ”Slow and Steady Wins the Race!”

Everyone who has cosmetic surgery or any type of surgery must allow appropriate time to heal and recover. We have busy lives, and many patients want their surgery and be healed instantaneously. This doesn’t happen!

If we don’t take time to heal, we can cause problems, increase risks, and delay healing. “Be the Turtle” is an expression referring to the fable of the tortoise and the haire (rabbit). In this story, the rabbit rushes ahead and then because he falls asleep, he loses the race. The expression refers to the idea that you need to go slow after surgery and take time to heal. Otherwise, you can actually cause problems and “lose” the race. The “race” is healing after surgery as quickly as possible, with no problems.

The expression “Slow and Steady Wins the Race” refers to the exact same concept. Follow the instructions given. Go slow and heal properly. Be wise. Don’t try and rush things or you will be the rabbit and lose.

4: ”Circle the Wagon”

This is an expression used by surgeons. To my knowledge it is mainly used by a limited number of plastic surgeons. I learned it from Dr. Thomas Whetzel, a professor at the University of California. It refers to surgical dissection. When dissecting anatomy, you have to avoid working in just “one spot” and instead dissect around the entire area to more safely expose the anatomy.

If you are trying to isolate a particular nerve or vessel at surgery, dissection in one location might lead you into a hole with limited visibility. If you dissect above, below, and around this spot (circle the wagon) you will more safely identify the surgical anatomy. Circling the wagon is the best way to dissect safely for surgery.

5: ”Exposure, Exposure, Exposure!”

This can be thought of like a realitor’s phrase of location, location, location. With surgery, proper exposure of the anatomy is critical to safely find and dissect nerves and blood vessels. It the surgeon has proper exposure, then he can see what anatomy to protect. This leads to less chance of injury to the surrounding anatomy.

The concept of exposure is similar to “circle the wagon.” The expressions are both related to ways the surgeon should operate or dissect. The surgeon must have all important vessels and nerves safely identified. This will minimize the chance of injury to the surrounding anatomy.

6: ”Twinkle in the Eye”

Clinical experience can be more important than lab values and X-rays. When recovering from surgery, it’s important to be able to tell if the patient is recovering well. Recovery has its ups and downs, but the trend should be gradual improvement and healing. Patients often want to know if they are doing well and how to tell if they are not. The doctor can often look at a patient and tell if things are ok or not. A famous surgeon, Dr. F. William Blaisdell, called it “the twinkle in the eye.” If the patient was doing better or healing, you could tell from experience. Better than a blood level or X-ray, often just “seeing” the patient could help the doctor tell everything was ok.

This experience comes from years of taking care of patients. That’s why I tell my patients it’s important to keep their follow up appointments. By seeing them, I can get a better handle on their healing and what needs to be done to recover the best. If there is any question or concern, I tell my patients, “Just come into the office.” By seeing them, I can tell if they have a twinkle in the eye. It also helps to see what is really happening and if there is a problem or not.

Dr Blaisdell was one of my mentors at the University of California. He was a surgical pioneer, expert, and Professor and Chairman of the department of surgery. He invented the extra-anatomical bypass and started the first surgical ICU in California. He also started the first trauma center in California. He was a major influence on my training and a hard work ethic.

7: ”Tincture of Time”

Sometimes during the healing process, a doctor can not tell for certain if there is a problem or not. An infection can sometimes take a while to manifest and with time it will be self evident. Treatment might be delayed to see if it was indeed a problem or not. This waiting peroiod can be called a “tincture of time.” A tincture is a word that in pharmacy means a medicine that is often alcohol based or containing a chemical. It can also mean a small dose, trace or tinge. A tincture of time means we are going to give it a small dose of time to see if it improves or not. Usually with time, the healing will be self evident or not. With a waiting period, the doctor will be able to better assess what is best. Typically no harm comes from a small wait. It is often a wise thing to do to minimize risks of unnecessary treatments.

This educational writing discusses insurance coverage of cosmetic and plastic surgery. Who will pay? What will be the cost? Is it going to be covered? Will insurance pay for my plastic surgery? These are fantastic questions that are normal, and they should be addressed for anyone considering these procedures. For the interested reader, please see my book – “A Woman’s Guide to the Real-Reality of Cosmetic Surgery.” The book reviews and discusses major concepts about cosmetic surgery, results, recovery, and expectations.

It is very common for people to come into my office and state, “My insurance company has told me that my tummy tuck operation will be covered.” Or they might have been told their eye lift or eye tuck operation will be covered because of blocked vision. The unfortunate truth is that the great majority of these cases are not covered by insurance companies, and the company is misrepresenting the truth. This article will cover some of the truths and facts about cosmetic surgery, plastic surgery, and insurance companies.

I am a Board Certified Plastic Surgeon, and I (like most plastic surgeons) want insurance to work for my patients. It helps pay your expenses and is a great thing when covered! The plastic surgeon is on the side of the patient. It is very important to understand that the surgeon is on “your side.” We want the insurance to work or pay for the cosmetic surgery. My staff assists patients with all the paper work necessary for insurance claims. This includes appropriate codes, documents, and forms. Common requests are for tummy tuck surgery, eye lift or eye tuck surgery, and breast reduction surgery. Also, insurance coverage for a nose job or rhinoplasty is often desired. I want the procedures covered, but it is critical to know that it is not that simple, and most of the time it will not be covered.

The truth is the insurance company is not giving the patient the whole picture when they say a procedure is covered. They should tell you that most of the time these procedures are denied and are only approved infrequently. It can really disappoint a person, because they are told this by the insurance company, and then they don’t get approved. The patient then feels they were “robbed.” Insurance companies are in the business of making money. They do this by “denying” surgery which makes more company profit by saving the expense of surgery. It can not be over emphasized that they make money by denying your surgery! It might be listed as a covered procedure, but this rarely is the case. It disappoints the doctor too, because he then uses valuable consultation time talking about insurance issues, instead of your medical needs and surgery.

Family doctors or other physicians will often tell a patient that the surgery is covered. Patients come in saying, “My doc told me my surgery will be covered.” Unfortunately, the doctor who said this isn’t the one trying to get the insurance coverage. This can lead to disappointment when the surgery is ultimately denied.

Some examples will help with this discussion. These are true stories. The first is a women who had lost 70 pounds from diet and exercise. She was interested in a tummy tuck. She was concerned about hanging skin and rashing at the folded over skin. She called her insurance company as was told that this is a “covered benefit.” Photographs were submitted with all appropriate codes. Her case was unfortunately denied. The insurance company deemed it a cosmetic abdominoplasty. They said the skin would have to hang down half way to the knees to be covered. Fortunately, the patient was able to have the surgery on a cosmetic basis. She healed well and was extremely happy with her results.

Another example was a women who wanted her breasts lifted and reduced. She complained of breast pain, neck and back pain, and shoulder grooving from her bra. She wore a 40 DDD. She was 5′ 6″ tall and weighed 155 pounds. Photos were submitted and codes given for a breast reduction. Unfortunately, she was denied by her insurance company who said it was cosmetic breast surgery. They said she had to be close to ideal body weight (within 10%) and should lose 20 pounds and resubmit. Also, they said one pound of tissue would have to be removed per breast, if she was successful in the weight lose. This would leave her as a B cup and she wanted a C cup. She also proceeded with cosmetic breast surgery, without insurance. She wanted to pick her own size and not be forced to go too small. She ended up extremely happy with her new breasts, which were C cup and “perky!”

It is intereting to understand how insurance companies really will fight to deny surgery. Patients might find it incredible that the companies considered cleft lip deformities in babies a cosmetic issue! Yes, this is true. We can thank our federal government who created a law stating cleft lip surgery is not cosmetic and mandated coverage. They forced the companies to accept that plastic surgery for cleft lip surgery is not cosmetic! If they were fighting this, you can bet they will fight tummy tuck or breast lift, or breast reduction surgery!

There are a few exceptions about cosmetic surgery and insurance coverage. If a person has had a gastric bypass then insurance coverage of the tummy tuck is easier, but it is not a guarantee. Also, rhinoplasty or nose job surgery and insurance companies can be a more positive experience. The septum or breathing aspect of the surgery can be covered. This is called a septoplasty. This is not the cosmetic aspect. However, the septum being approved can really offset the expenses of the surgery. Finally, eye lift surgery or blepharolasty surgery can be covered if a person fails the visual field test. This is a test that demonstrates major blocking of vision by hooding. This usually has to be severe however.

It also is extremely difficult to get thigh lift surgery, buttock lift surgery, or lower body lift surgery covered, even after gastric bypass surgery and/or extreme weight loose. My office will try and fight for you, but you must remember it is difficult to win. They make money by denying surgery!!

Men are interested in the benefits of cosmetic surgery and plastic surgery in increasing numbers. Male patients can benefit from liposuction, liposculpturing, and other cosmetic operations. It is natural for men to want to look thinner, flatter, and younger. It is natural to want to turn the clock back and have a more fit, thin, and sculpted abdomen, chest, or love handles.

Cosmetic procedures can help men from Delaware, Media, Philadelphia, West Chester Pa and other geographic areas achieve these goals! Please see the before and after photos for male liposuction.

Sometimes a male patient can not achieve the sculpted abdomen or stomach area that he wants. Exercise alone doesn’t always give the results that we hope to achieve. Diet, exercise, and proper nutrition are always important but might not give the final desired appearance. Liposuction or liposculpting can help a person who wants further improvement. Male liposuction surgery or liposuction for men does not replace the importance of diet and exercise. However, the cosmetic procedures are an option for those who want more. The surgery can really give the person a look that was not achieved with working out alone. The plastic surgery can be a big help for many people, giving them the extra fit look.

I specialize in male cosmetic liposuction or liposuction for men. I use the latest techniques including tumescence and liposculpturing to give the best results. Liposculpting is the process of removing the fat with the smallest cannulas or instruments possible. It is believed this removes the fat in the best manner to get a result that is the smoothest and with the least risks.

What areas can be treated?

Liposuction can be done to “suck out” and remove excess fat from the abdominal area or stomach. It is commonly done also to the flanks, or love handles. These two areas are some of the most frequently performed operations on men. The chest area is also frequently done. Gynecomastia is the development of “man boobs” or fat deposits in the chest. Treatment can help remove unwanted fat from this location too.

What about technique?

As mentioned above, I believe the tumescent technique and liposculpturing give the best results. The goal is the smoothest removal of fat with the least risks of an unnatural shape or lumpy appearance. Another technique that I perform includes “etching” the abdominal area to try and achieve a more sculpted appearance to the stomach – like creating a six-pack look!
Liposuction results are improved with the tumescent technique, liposculpting, and the “etching” to achieve an enhanced result! I have said that almost every man can benefit from liposuction of the stomach, flanks, and love handles: including myself!

What about recovery?
Liposuction recovery can be surprisingly easy for most male patients having cosmetic surgery. The liposuction recovery is usually just 2-4 days off work or just a long week-end. Most patients are surprised at how little pain is involved! Compression garments are worn after surgery to help with liposuction recovery and bruising. These can be hidden under most clothing.

What about risks?

All operations have risks. The risks overall are considered very low. It is important to understand realistic expectations. This means surgery can only do so much. Even with the best techniques, a person doesn’t want to expect more than what really can be achieved with the procedures.

I will work closely with you to determine you specific goals for your liposuction surgery and other cosmetic needs. During the consultation, I will review liposuction cost, recovery, technique, and other important aspects of the cosmetic operation.

I am a board certified plastic surgeon who practices exclusively cosmetic surgery. Please check out the liposuction before and after photos on our web site. Also, please come in for a complimentary consultation to learn about your choices and options!

Male patients are seeking cosmetic surgery and plastic surgery in ever increasing numbers. As reviewed in Part One, men have normal desires to look younger, thinner, and healthier. It is common to want the changes and improvements that cosmetic surgery and plastic surgery for men can give people. This writing will focus more on facial rejuvenation. People from all over and including Delaware, Media, Philadelphia, and West Chester PA will enjoy the information. Please see another educational writing for additional information. It is titled “Facial Rejuvenation Concepts: Facelift Delaware: Eye Lift Delaware.”

Male cosmetic and plastic surgery includes facial surgery like eye lid lift or blepharoplasty, facelift or rhytidectomy, nose job or rhinoplasty, and facial fillers, Botox or Dysport. Men also want to look thinner and flatter and are interested in liposuction or liposculpturing. Some men also benefit from tummy tuck surgery or abdominoplasty cosmetic surgery. For those interested, please see my video (Eyelid surgery with Dr. Saunders) that demonstrates an eyelid lift. It shows the skilled and meticulous nature of the surgery.

With facial rejuvenation surgery for men, patients want to look younger and improve jowls, droopy cheeks, and neck excess. I specialize in male cosmetic surgery and help my patients achieve their goals. Men do not get the same “facelift” as a women patient. The male facelift operation is adjusted and tailored to a male’s anatomy and goals. There are many differences. This includes the thickness and quality of the skin. The differences include also the blood supply, hair, and the hair pattern. Finally, there are differences in the glands, bones, muscles, and deeper tissues. These factors should be considered to give the best facial rejuvenation for the male patient.

For example, in a facelift or neck lift for men, the technique is adjusted to consider a male’s hairline. It is different from a women’s operation. The incision can be hidden behind the tragis of the ear to give an “extra hidden” incision. It is also adjusted in the temple area to give a male pattern in this location. I use the latest techniques, in my opinion, for my male facelift cosmetic operation. A facelift for men often includes SMAS tightening and “deeper muscle” work to give the best rejuvenation possible.

Facelift recovery is surprisingly easy! Most male patients are surprised at the very small amount of pain. Often, people don’t even need pain pills or just a few! However, the male cosmetic patient needs to remember that bruising and swelling take time to resolve. Time off from work needs to be considered after this surgery. Computer work, telephone calls, and desk work can usually be resumed in just 2 or 3 days. Business men can be surprised at how quickly they get back to their office work. A longer amount of time is necessary before one goes out in public or does business calls. The recovery can be summarized as being surprisingly easy with minimal pain. However, bruising and swelling is on the face and is visible. This takes time to resolve and go away. An appropriate amount of time, such as a week or two, is needed for this healing phase.

I will work closely with you to determine you specific goals for your facelift or rhytidectomy and other cosmetic needs. During the consultation, I will review facelift cost, facelift recovery, facelift technique, and other important aspects of the cosmetic operation.

Please call the office for a complimentary consultation. Also see the facelift before and after photos in the photo gallery in the menu.

Delaware, Pennsylvania, Maryland, New Jersey – Part One

Men make an ever growing percentage of people who are interested in plastic surgery and cosmetic surgery. I work closely with my male patients to give them the best care possible. Statistically, men can make up 10% of many plastic surgery practices. Men have the same natural desires, just like everyone else. Men also want to look good, feel better about themselves, and to look younger. It is normal for men to have these desires. It is normal to want to look your best! Patients from Delaware, Pennsylvania, Maryland, and New Jersey come to see me. Please see my before and after photos for men.

According to the American Society of Plastic Surgeons, men had about 750,000 cosmetic procedures, almost 8% of the total last year. This was an increase of over 88% from a decade ago. The top five surgical procedures for men are: liposuction, rhinoplasty, eyelid and facelift surgery, breast reduction to treat enlarged male breast, and cosmetic ear surgery.

Just as it is normal to eat a proper diet, exercise, and dress well, it is normal to desire a more attractive appearance or younger look. The truth is- almost everyone feels this way! People who say otherwise often are not bein honest with themselves. Others may not have enough extra money for cosmetic surgery, so it is easier to just say that it isn’t what they want.

Men have sometimes found that they can lose business to younger male competition, because they are older looking. Many plastic surgery and psychological studies published in the literature have shown very interesting results. If we look older, we are viewed by others as being less intelligent and successful! This might be hard to believe, but it’s true! People do judge us by the way we look – even if they shouldn’t- it happens.

Cosmetic surgery for men can be extremely successful and usually is rewarding. Men have different anatomy and surgical goals than women. I am an expert in these differences and will help my male patients with their desires and wants.

For example, aging of the eyes typically leads to excess skin and fat above the eyelids. This makes us more tired and older looking. The treatment is an eyelid lift or blepharoplasty. The latest way to do this is to treat all three levels of aging. This includes the skin, muscle, and the fat. With this most advanced technique the eyelid lift or eyelid tuck give a natural and youthful appearance.

However, with men, if you remove too much skin and fat with the eyelid lift or blepharoplasty, you run the risk of being “feminized.” Women have a different anatomy above the eyes and the skilled plastic surgeon needs to address this. By knowing the difference, the male patient can get a more youthful and healthy appearance without the feminization sometimes seen. (Michael Douglas, the movie star, had too much fat removed at his first operation and looked unnatural.)

Recovering from eyelid lift surgery or blepharoplasty surgery is surprisingly easy. Blepharoplasty recovery is characterized by very little pain! Most people do not need any pain meds! However, there is bruising and swelling that slowly goes away. The blepharoplasty cost is also surprisingly affordable. The operation for eyelid lift can be extremely successful and with very low risks. Please see my before and after photos on eyelid and facial rejuvenation.

In addition to blepharoplasy, Dr. Saunders works closely with his male cosmetic surgery patients to help them address all their needs. From liposuction for men to facelift for men, he will help you with your goals and wants.

I specialize in cosmetic surgery and will work closely with you for your best results. Please call the office for a complimentary consultation. Also, please see the before and after photos for male patients in the photo gallery!

Media, Delaware Valley, PA
Part Two – The Concept of Soft Tissue Coverage

As covered in Part One, breast lift operations give a certain shape and appearance that is different from breast augmentation or breast enlargement. This different result is extremely important to understand. During consultations, I spend the time to teach and explain these differences to my patients. People from Delaware Valley or Media, Pennsylvania and Newark or Wilmington, Delaware and surrounding states come in to learn the facts about these operations. Education about these cosmetic operations will help a woman understand and be more satisfied with her procedure. Please also see my educational writing “Breast Augmentation, Breast Enlargement: What is Settling?”

One can think of the changes with breast lift or mastopexy and the changes with augmentation as being on a gradual curve or slope. It is like a mathematical line that is gradually increasing as you go from the left side to the right side. It can be thought of as being a spectrum with breast lift results on one side and breast augmentation results on the other side.

The results of a lift are on the most left hand side or the beginning of the curve or line. Small breast implants will give a result that is slightly to the right. This is further up the line. Medium implants put you still further up the line. Bigger implants push you further to the right. Finally, huge breast implants push you to the far right on the curve or line.

What is changing? The result of the operation changes. The shape of the breast changes. The “look” changes. Simply put- a breast lift gives the breast a more perky and natural shape, but breast implants give extra roundness and fullness on the top aspects of the breast. When you understand this difference, you will understand breast implants versus breast lift.

A mastopexy or breast lift makes the breast perkier, lifted, and more attractive in shape and appearance. However, there will be less volume in the upper aspect of the breasts. As we start adding a breast implant, it will add volume to the breast and give more of an “implanted look.” This is not necessarily fake! As a matter of fact, most people do not think small breast implants are fake looking. Small implants can be extremely attractive and natural. Small sizes are available to give a half cup or other small and natural breast augmentation results. Small and natural appearances can be achieved with implants.

As the implants get bigger, the shape and appearance becomes more round and full up top. The appearance now might be the classic augmented breast that many women like. It still is not necessarily fake. Beauty is in the eye of the beholder. Whether a breast looks fake or not is partially dependent on the woman. As the implants get even bigger, the shape gets even more round and full. Becoming “fake” definitely increases as the implants get extra big. This appearance tends to be really round and extra full. Please see my video on choosing implant size.

Why does the shape of the breast change with different operations? Why does the shape of the breast change with different size breast implants? The answer is the concept of “soft tissue coverage.” This is the tissue that hides or camouflages the breast implant. Soft tissue is the breast and the muscle that covers the implant. It’s soft and not the hard tissue like bone and ribs. The soft tissue covers the implant and makes seeing the roundness and the edges of the implant more difficult.

Understanding soft tissue coverage is very important. Small implants will be hidden well by the soft tissues, and the result will be more natural. Small implants will be hidden well, so it’s less round and full up in the top of the breast. Large implants will be hidden less well, and the shape of the breast will change. Bigger implants give a rounder and fuller shape or appearance. As the implants get bigger, the result is further to the right on the line or curve discussed above.

The amount of breast tissue you begin with plays an important role. The more tissue you have to start with means you have more soft tissue coverage. Flat chested women don’t have as much tissue to hide the implants. It will be harder to camouflage the implant because there is less to hide it. The flat breasted women might choose smaller implants if she is concerned about appearing too round or fake.

Placing the implants beneath the muscle provides extra soft tissue coverage and more natural results. This muscle coverage leads to softer and more attractive breast enlargement results. It also leads to lower complications.

Breast lift results are almost always more natural in shape, because there is no implant to hide. Results are extra natural with lifts. They might not be round enough or full enough compared to implant operations. If this rounder and fuller look is desired, than breast implants should be considered. If a patient gets a lift and after healing wishes the shape was bigger or fuller, she can always safely add implants at a later date. This is safe to do and is an option for the woman who isn’t sure about getting implant or not.

In summary, the shape of the breast changes from a lift to augmentation. Breast lift results are very natural. The operation makes the breast fuller and perkier. Small breast implants give more roundness and more fullness than a lift. This is seen in particular in the upper parts of the breast. Large implants give even more roundness and fullness. Fake results from implants come from extra large implants where you can really see the roundness of the implant. The concept of soft tissue coverage helps understand these changes.

People interested in breast augmentation or breast enlargement or other topics such as breast augmentation cost, breast augmentation recovery, or breast augmentation technique should please call the office for an appointment.

I am a board certified plastic surgeon who practices exclusively cosmetic surgery. Please check out the breast augmentation before and after photos on our web site. Also, please come in for a complimentary consultation to learn about your choices and options!

Media PA, Delaware Valley
Part One:

Breast augmentation, breast enlargement, or implants give different results, shape, or appearance than a breast lift operation. Patients seeking cosmetic improvement in their breasts need to understand these differences. The results are not the same. The outcomes are different. We don’t want to be disappointed by getting different shapes or results than desired. We don’t want to pick the wrong operation. People in Media or Delaware Valley PA or the surrounding areas and states can learn and understand these important facts. For those interested, please also see my educational writing on Breast Augmentation Results = “A Bigger You.”

I have patients who have come in to the office and they say, “I dont like the results of a breast lift or mastopexy. I’ve been all over the web, and I don’t like any doctors breast lift pictures. The result and shape are not what I want. They look flat and not round enough.” Yes, this can be true and is an opinion that can be valid. They might be looking at good breast lift results and just don’t like the shape. They might need a different operation or breast implants instead. It doesn’t mean that a woman won’t be happy with a lift, but she does need to understand the different shapes and results achieved by the two operations. She needs to understand breast augmentation vs. lift.

Americans are used to seeing women in the movies and magazines, such as Playboy. People have come to view these women as having normal or natural breasts. But Mother Nature doesn’t make breasts like this. Normal breasts don’t look like this. These women almost always have implants. In the movies and Playboy, even the smaller breast shapes tend to be women with breast implants – just smaller ones! It is not a normal shape but a breast implant shape. Learning what natural breast augmentation results look like is important.

Breast implants don’t just make the size bigger. It’s more than just a bigger size. Breast implants tend to be rounder and more full in the entire breast, but especially above the nipple in the upper aspects of the breast. The result is rounder, bigger, and perkier too.

With smaller breasts, the appearance can be deflated and droopy. The breast might not actual droop. It just might be partially empty and small, giving it this appearance and shape. Breast implants can give fill the breast up and enhance it’s appearance, giving it a lifted look.

Breast augmentation involves placing breast implants beneath the Pectoralis muscle and breast tissue to enlarge the breast. The shape or result achieved is determined by the size of the implants and the nature of the tissue on top of it.
I have made a video that reviews different breast enlargement results with different breast implant sizes.

What about a breast lift? Breast lift or mastopexy surgery reshapes the breast and elevates the nipple. The operation gives a more natural, perky, attractive, and full appearance to the breast. But, it doesn’t give that extra round or full shape that implants do. Breast lifts tend to give a shape that is more natural or normal, like Mother Nature makes. The breasts are more straight or flat up top in the upper parts of the breast. They become more perky and full than before surgery, but just not the same fullness as with implants.

Breast lift surgery elevates the breast and gets hanging tissue beneath the crease of the breast a lift. It gives the breast a wonderful and improved shape and appearance. This shape is more full and attractive. Most woman are very happy and pleased with the result of their lift. However, it’s not as round or as full as with implants. This is an important distinction between breast implants versus breast lift.

Once a woman understands the differences between the operations, she should work with her doctor to decide the best operation for her. Sometimes this means doing implants and not a lift. This might be the case if more roundness and fullness is wanted. Sometimes this will mean doing a breast augmentation and breast lift, if hanging and small volume are both present.

I work closely with my patients to help them understand the operations and results. By thorough discussion and examination, I can help the woman learn about the procedures. This allows her to be more comfortable and happy with her final decision.

Many women struggle with breasts that have begun to sag and droop. This is happens for many reasons. Breast feeding, changes in weight, genetics and the natural effects of aging all take their toll. Dr. Chris Saunders has helped countless women restore their breasts to create a more youthful appearance through Wilmington Breast Lift surgery.

In a Breast Lift procedure an incision is made around the areola and excess, sagging skin is removed. The areola may also be repositioned, reshaped, or re-sized to create the most ideal results. The remaining skin is then closed creating firmer, perkier, more youthful breasts. In some cases, a breast augmentation may be coupled with a breast lift procedure.

If you would like to learn more about the breast lift procedure and to find out if you are a good candidate, contact our office to schedule a consultation with Dr. Saunders.
